*BlumeHaus Physical Therapy operates primarily as an out-of-network, private care practice. This model allows us to deliver unrushed, one-on-one treatment tailored entirely to you—without the limitations often imposed by insurance-based care. You will work exclusively with your physical therapist for the full duration of each visit, without technicians or aides, supporting more efficient progress and a higher standard of care. Many plans include out-of-network benefits. Upon request, we are happy to provide a detailed superbill for potential reimbursement. We also accept HSA and FSA cards for your convenience. If you wish to utilize one of our select in-network plans (currently Blue Cross of Idaho), please share your plan details below and our team will personally follow up to assist with next steps.
